Puntos claves de la nueva guía para la prevención, el diagnóstico, la atención y el tratamiento de personas con infección crónica por VHB

La infección crónica por el virus de la hepatitis B (VHB) continúa siendo un problema relevante de salud pública, con alta prevalencia y una carga importante de cirrosis y carcinoma hepatocelular. A pesar de las estrategias globales orientadas a su eliminación, persisten barreras importantes como el diagnóstico, criterios terapéuticos restrictivos o acceso limitado al tratamiento, especialmente en países de ingresos bajos. En respuesta a estos desafíos, y a la luz de evidencia reciente que demuestra un riesgo de carcinoma hepatocelular en pacientes con carga viral ≥2000 UI/mL, incluso en ausencia de cirrosis o elevación del nivel sérico de enzimas hepáticas, la Organización Mundial de la Salud publicó en 2024 una nueva guía que actualiza el abordaje clínico de la infección por VHB. Estas recomendaciones incluyen la ampliación de los criterios terapéuticos en adultos y adolescentes, el fortalecimiento de la prevención en mujeres embarazadas para reducir la transmisión maternoinfantil, la definición de nuevos puntos de corte para métodos no invasivos de evaluación del daño hepático, como índices séricos y elastografía, y la simplificación de las pruebas diagnósticas con el fin de facilitar la detección temprana y la vinculación oportuna a la atención. En esta revisión se presenta una síntesis actualizada de la hepatitis B, desde su epidemiología e inmunopatogénesis hasta su historia natural, y se analizan los cambios fundamentales introducidos por la guía de 2024, con énfasis en un enfoque equitativo, la simplificación de los procesos y la ampliación del acceso al tratamiento.
