Linfoma de Hodgkin con afectación hepática primaria: más allá de la presentación habitual

El linfoma de Hodgkin (LH) es una neoplasia maligna de origen linfoide, que afecta con mayor frecuencia los ganglios linfáticos, pero puede extenderse al bazo, el hígado, la médula ósea y otros órganos. La forma de presentación más frecuente consiste adenopatías cervicales no dolorosas. La expresión clínica inicial del LH con compromiso hepático colestásico es poco común y, cuando se manifiesta, es en estadios avanzados de la enfermedad. Se presenta el caso de una paciente de 61 años con antecedente de hepatitis B crónica, quien consultó por fiebre prolongada y pérdida de peso, hiperbilirrubinemia y hepatomegalia palpable. La colangioresonancia mostró múltiples lesiones focales hepáticas sugestivas de neoplasia. Ante la sospecha de malignidad, se realizó una resección hepática en cuña, cuyo estudio histopatológico confirmó el diagnóstico de linfoma de Hodgkin primario de hígado. La evolución posquirúrgica fue desfavorable, con desarrollo de insuficiencia hepática fulminante asociada a encefalopatía, coagulopatía severa y disfunción multiorgánica, con desenlace fatal a las 48 horas de su ingreso a la unidad de cuidados intensivos.
